Summary
Michigan is launching a new Targeted Case Management (TCM) program for children with medical complexity, effective February 1, 2025. Action Required
By February 1, 2025: Providers interested in offering CMC TCM services must enroll in Medicaid as a CMC TCM case management entity and obtain certification from MDHHS. Set up processes to identify eligible children (under 21 with CSHCS conditions involving 3+ organ systems, functional limitations, and high utilization). Implement referral workflows and obtain required forms (BPHASA-2412, BPHASA-2409, BPHASA-2410) from the upcoming MDHHS website. Verify authorization in CHAMPS system before billing any CMC TCM services as Fee-for-Service.
