Wisconsin Medicaid now requires providers to submit a copy of the child's current Individualized Family Service Plan (IFSP) with all prior authorization requests for children aged birth to 36 months. Additionally, the Newborn Report Form must now be sent to a new address.

If IFSP already submitted with previous PA, reference that PA number. If no IFSP exists, provide written justification.
