Summary
Wisconsin Medicaid has redesigned all provider certification packets and will only accept new packet formats beginning September 1, 2001. The policy also introduces new web-based search capabilities and fee schedule download formats.
Action Required
By September 1, 2001: Wisconsin Medicaid providers must obtain and use only the new certification packet formats from Wisconsin Medicaid Provider Services at (800) 947-9627 or (608) 221-9883. Old certification packets received after this date will be returned with a 30-day extension to resubmit using the new format.
