Immediately: Staff must verify eligibility for ALL Medicaid Forward cards presented by foster care children using the Eligibility Verification System (EVS). For deactivated cards (rejection code 79), manually enter recipient's 10-digit Medicaid ID, name/SSN, DOB/SSN, or name/DOB on card reader keypad. For defective cards, use Personal Account Number from card front or call Provider Services at (800) 947-9627. Delay claim submission for a few days after service date to allow eligibility updates. Make copies of green temporary cards for Good Faith claims processing.
