Action Required
Before October 2008: Complete and submit new Wisconsin Medicaid Provider Agreement and Acknowledgement of Terms of Participation. Create ForwardHealth Portal account to continue electronic business with BadgerCare Plus and access secure Portal areas. Trading partners must complete Trading Partner Profile HCF 13043 and conduct testing beginning June 2, 2008. Failure to establish accounts will prevent electronic transaction processing.
