Summary
ForwardHealth (Wisconsin Medicaid/BadgerCare) now requires providers to use only the 1500 Health Insurance Claim Form (version 08/05) or UB-04 claim forms. The older CMS 1500 and UB-92 forms will no longer be accepted and claims using these forms will be denied or returned unprocessed.
Action Required
By August 1, 2008: Billing team must immediately stop using CMS 1500 and UB-92 claim forms for all ForwardHealth submissions. Switch to 1500 Health Insurance Claim Form (version dated 08/05) or UB-04 claim forms only. Update billing software and processes to use new forms. Continue to include Medicaid provider identification numbers on claims. Obtain new forms from federal form vendors as ForwardHealth does not provide them. Claims submitted on old forms will be denied or returned unprocessed.
