By October 2008: All Medicaid-certified providers must complete and submit a new Wisconsin Medicaid Program Provider Agreement to conduct business via the ForwardHealth Portal. Contact Provider Services at (800) 947-9627 to verify completion or request a new copy. Begin establishing Portal accounts at www.forwardhealth.wi.gov/ starting August 18, 2008. Update billing workflows to use the new portal for claims submission, prior authorization requests, and enrollment verification.
