Summary
Wisconsin ForwardHealth is implementing new imaging software in October 2008 that requires all paper forms to contain barcodes for processing. Providers must use only official ForwardHealth-issued forms (both mandatory and voluntary) as forms without barcodes cannot be processed by the new system.
Action Required
By October 2008: All staff must use only official ForwardHealth-issued forms with barcodes for all paper submissions including claim adjustment requests, prior authorization forms, and hospice election forms. Replace any altered or copied versions of forms with official ForwardHealth versions. Forms without barcodes will not be processed by the new imaging software.
