Action Required
By October 2008: All Wisconsin Medicaid providers must transition to new certification materials and applications for ForwardHealth interChange system. Billing teams should update provider certification processes to use either the new online ForwardHealth Portal at www.forwardhealth.wi.gov or request new paper applications. Old applications received after implementation will be returned unprocessed, but initial effective date remains valid if new completed application is returned within 30 days.
