Summary
ForwardHealth (Wisconsin Medicaid programs) is implementing a new system in November 2008 that requires all healthcare providers to use National Provider Identifiers (NPIs) instead of current 8-digit provider numbers when billing Medicaid, BadgerCare Plus, SeniorCare, and other Wisconsin state programs. Providers must report their NPIs to ForwardHealth before the system goes live or claims will be denied.
Action Required
Before November 2008: All providers must obtain appropriate Entity Type 1 or Type 2 NPIs and report them to ForwardHealth using form F-13505. Complete and submit the National Provider Identifier Collection form by fax at (608) 221-2163 or mail. Update billing systems to use NPIs instead of 8-digit provider numbers for all Wisconsin Medicaid program claims. Include required taxonomy codes and ZIP+4 codes when NPI corresponds to multiple certifications. Failure to report NPI will result in claim denials and payment delays.
