Summary
Wisconsin ForwardHealth providers can now designate a separate address specifically for prior authorization correspondence through the ForwardHealth Portal or form F-1181. Previously, PA responses were sent to addresses indicated on PA requests, but now they will go to the practice location address unless a separate PA address is designated.
