Summary
ForwardHealth will now generate 1099 forms only for earnings over $600 (per IRS regulations) instead of for all earnings, and will send separate forms per financial payer and tax ID number. Forms will be mailed to the designated '1099 mailing address' in the system.

Verify that your practice's correct 1099 mailing address is on file with ForwardHealth by completing Form F-1181 if needed. Expect separate 1099 forms for each program (Medicaid, BadgerCare Plus, etc.) if your practice participates in multiple ForwardHealth programs.
