Summary
Women diagnosed with breast or cervical cancer while enrolled in BadgerCare Plus Benchmark or Core Plans are now eligible to enroll in Wisconsin Well Woman Medicaid, which provides comprehensive Medicaid coverage without copayments. Providers must complete form F-10075 with the patient to initiate enrollment, and women enrolled from HMOs will switch to fee-for-service reimbursement.
Action Required
Immediately: Identify women diagnosed with breast or cervical cancer enrolled in BadgerCare Plus Benchmark or Core Plans. Complete and submit Wisconsin Well Woman Medicaid Determination form F-10075 with patient signatures to dhsemcapo@wisconsin.gov or fax to (608) 267-3381. Billing team must update patient insurance information once WWWMA enrollment is processed (within 10-30 days) and reimburse patients for any copayments paid after diagnosis date while still on Benchmark/Core plans.
