Summary
BadgerCare Plus Basic Plan members diagnosed with cancer (excluding non-melanoma skin cancers) can now bypass the Core Plan waitlist for immediate enrollment. Providers must complete form F-00292 and submit it to the Enrollment Services Center to facilitate the transition.
Action Required
Immediately: Providers treating BadgerCare Plus Basic Plan members with cancer diagnoses must complete Core Plan Waitlist Medical Bypass Determination form F-00292, including cancer type description. Submit completed forms by fax to (888) 415-2115 or mail to Enrollment Services Center. Keep copies in patient medical records. Note: Non-melanoma skin cancer patients are not eligible for this bypass process.
