ForwardHealth's fiscal agent is changing their mailing address from 6406 Bridge Road to 313 Blettner Blvd, Madison WI 53784, effective April 23, 2012. All mail previously sent to Bridge Road should now be sent to the new Blettner Boulevard address, though mail forwarding will be temporarily available.

By April 23, 2012: Update all mailing addresses in billing system and correspondence to use ForwardHealth's new address: 313 Blettner Blvd, Madison WI 53784. Replace any references to 6406 Bridge Road. Continue using existing P.O. Box addresses until further notice - do not substitute the street address for P.O. Box mailings as this may cause processing delays.
