Summary
ForwardHealth is implementing new provider enrollment and screening requirements for Wisconsin Medicaid and BadgerCare Plus programs due to the Affordable Care Act. The enrollment process will become completely paperless, require additional screening information, and providers will be assigned risk levels with corresponding application fees for certain provider types.

By August 13, 2012: Providers enrolling in Wisconsin Medicaid/BadgerCare Plus must complete enrollment entirely through the online Portal (no paper applications accepted). Prepare additional information for persons with ownership/control interest, managing employees, and agents. Ordering and referring physicians must enroll as participating Medicaid providers. Monitor for fall 2012 implementation of risk-level assignments and application fees up to $523 for certain provider types.
