By August 17, 2012: Front desk staff must inform affected HMO patients in Milwaukee, Waukesha, Washington, Ozaukee, Kenosha, and Racine counties that they must call LogistiCare directly to schedule non-emergency medical transportation starting September 1, 2012. Update patient education materials to include LogistiCare contact information for transportation coordination.
