Summary
Starting December 6, 2013, Medicaid-enrolled providers in Wisconsin must update their demographic information (addresses, phone numbers, tax IDs, etc.) through the ForwardHealth Portal's online demographic maintenance tool instead of using paper forms. The previous Provider Change of Address or Status form (F-01181) is discontinued and any paper forms received after December 6 will be returned.
Action Required
By December 6, 2013: Wisconsin Medicaid-enrolled providers must stop using paper Form F-01181 for demographic changes. Access the ForwardHealth Portal and use the Demographic Maintenance tool (found in the Home Page box) for all updates to addresses, phone numbers, tax IDs, licensure, ownership, and other demographic information. Administrative or clerk accounts with Demographic Maintenance role can make these updates. Failure to notify ForwardHealth of changes may result in incorrect reimbursement, misdirected payments, claim denials, or payment suspension.
