Summary
ForwardHealth (Wisconsin Medicaid) now requires all healthcare providers and partners to use only the online ForwardHealth Portal for Express Enrollment applications. Paper applications are no longer accepted as of November 11, 2013, and address changes must be reported through new online tools effective December 2013.
Action Required
Immediately for Wisconsin providers: If participating in BadgerCare Plus or Wisconsin Medicaid Express Enrollment, transition all enrollment applications to the ForwardHealth Portal online system at www.forwardhealth.wi.gov/. Stop using paper applications. For address changes, use the new online Express Enrollment Change of Address tool accessed through the Express Enrollment Change Request link in provider accounts.
