Wisconsin Medicaid now requires fingerprinting and criminal background checks for high-risk providers and owners with 5% or more ownership interest, effective retroactively from August 1, 2015. Providers have 30 days to complete fingerprinting or face enrollment denial/termination.
Action Required
Within 30 days of notification: High-risk providers and any owners with 5% or more ownership interest must schedule fingerprinting appointments at www.fieldprintwisconsin.com using their Fieldprint code and ATN. Pay $7.75 fee and complete fingerprinting process. Contact Fieldprint at 877-614-4364 for appointment questions. Failure to complete fingerprinting within 30 days will result in Medicaid enrollment denial or termination.
