Summary
Starting January 1, 2017, Wisconsin provider organizations that previously paid application fees for Medicaid enrollment must now also pay a $560 application fee when revalidating their Medicaid enrollment every few years. Individual providers and professional groups are exempt from this fee.

Before revalidation due date: Provider organizations subject to application fees must budget $560 for Medicaid revalidation and ensure payment is submitted within 10 business days of application submission through the ForwardHealth Portal. Update contact information via Portal demographic maintenance tool to ensure revalidation notices are received. Failure to pay fee within 10 days will result in application denial and potential enrollment termination.
