For Harvard Pilgrim products, submit changes via CAQH Provider Data Portal or email Provider Change Forms to PPC@point32health.org. For Tufts Health Plan products, email changes to provider_information_dept@point32health.org. Facilities must complete attestation forms on HPHConnect or Tufts secure provider portal. Failure to update within 90 days will result in directory suppression until information is validated.
