Summary

Action Required
Every 90 days: Administrative staff must review and verify all practice information in Harvard Pilgrim and Tufts Health Plan provider directories, including practice location, phone number, hours of operation, and provider availability for new patients. Use CAQH Provider Data Portal for individual practitioner updates. For contractual changes, submit Provider Change Form to PPC@point32health.org (Harvard Pilgrim) or Medical/Behavioral Health Provider Information Form to provider_information_dept@point32health.org (Tufts). For facilities, complete attestation forms via HPHConnect or Tufts secure provider portal. Failure to update information will result in directory suppression affecting patient access and referrals.
