Summary

Starting May 1, 2026, Harvard Pilgrim Health Care Commercial and Tufts Health Direct will require prior authorization for ALL diabetic GLP-1 and GIP/GLP-1 medications (Mounjaro, Ozempic, Trulicity, etc.), removing the current automated step therapy program. Previously, these medications were automatically covered if patients had prior oral diabetes medication claims within 365 days.

Before May 1, 2026: Billing team must update prior authorization workflows for all Harvard Pilgrim Commercial and Tufts Health Direct members requesting diabetic GLP-1 and GIP/GLP-1 medications (Bydureon BCise, Byetta, Mounjaro, Ozempic, Rybelsus, Trulicity). Providers must submit requests through medical review process with documentation of type 2 diabetes diagnosis and trial/failure of oral hypoglycemic agent. Update encounter forms and EMR templates to prompt for required documentation. Claims will be denied without proper prior authorization.
