Action needed
Immediately: Billing team must review current PEN and EN nutrition claims to ensure compliance with new medical necessity criteria. Update prior authorization requests to include required documentation: for PEN - evidence patient receives <30% calories orally, wasting indicators (weight loss >5% in 1 month or >10% in 6 months, albumin <3.4 gm, BUN <10 mg, phosphorus <2.5 mg), and qualifying conditions. For EN - physician orders specifying nutrients and administration method, documentation of functional impairments, and confirmation formula provides >75% of daily calories when given orally. Train staff on excluded items including food allergies formulas, conventional low-protein foods, probiotics, and supplemental nutrition products.
