Stop submitting claims for CPT codes 64555, 64585, 64590, 64595, 64596, 64597, 64598 and related HCPCS codes C1767, C1778, C1816, C1883, C1897, C9807, L8679, L8681. Notify providers that peripheral nerve stimulation devices are not covered and require patient self-pay arrangements before procedures.
