Summary
Blue Cross Blue Shield maintains that automated percutaneous discectomy and percutaneous endoscopic discectomy are investigational procedures for treating disc herniation. Two new CPT codes (62330 and 62331) for percutaneous ligamentum flavum decompression are now considered investigational, and existing CPT codes 62287 and 0274T have revised descriptions effective January 1, 2026.
Action Required
Before January 1, 2026: Billing team must update system to flag CPT codes 62267, 62287, 62330, 62331, 62380, 0274T, and 0275T as investigational/non-covered for Blue Cross Blue Shield plans. Update code descriptions for CPT 62287 and 0274T in billing software. Notify providers that these percutaneous and endoscopic discectomy procedures will not be covered and require alternative treatment approaches or patient financial responsibility discussions.
