Summary
Blue Cross Blue Shield of Mississippi updated their non-long acting insulin policy to add four new insulin mix products to the prior authorization requirements and explicitly state that prior authorization is now required for all non-Novolog/Novolin insulin products. Fiasp (aspart) was also added to the covered product list.

By February 24, 2026: Billing team must update prior authorization workflows for BCBS Mississippi patients requiring non-long acting insulin products other than Novolog and Novolin families. Add Humalog Mix, Humulin 70/30, Novolin 70/30, Novolog Mix 70/30, and Fiasp to the covered product list in billing system. Ensure providers document clinical evidence of Novolog/Novolin ineffectiveness or adverse reactions for prior authorization requests. Claims for non-preferred insulin products will be denied without proper prior authorization.
