Oregon Health Plan (OHP) now requires prior authorization for non-preferred thyroid hormone medications including levothyroxine capsules, levothyroxine solution, thyroid pork tablets, and liothyronine tablets effective October 1, 2022. Levothyroxine sodium tablets remain the preferred product without prior authorization requirements.

Before October 1, 2022: For OHP fee-for-service patients on non-preferred thyroid hormones (TIROSINT, THYQUIDITY, TIROSINT-SOL, ARMOUR THYROID, NP THYROID, CYTOMEL), providers must either switch patients to preferred levothyroxine tablets if medically appropriate, or submit prior authorization requests including all relevant ICD-10 diagnosis codes and documentation explaining why preferred options are not suitable. Claims will be denied without approved prior authorization.
