Action Required
By April 20, 2023: CCOs must submit completed Improvement Plans (IP) and evidence of implementation to Health Services Advisory Group (HSAG) addressing all Partially Met or Not Met compliance findings from 2020-2021 review cycles. The IP must identify specific actions taken, responsible individuals, and completion timelines. Failure to resolve findings may result in OHA sanctions per Contract Exhibit B, Part 9, Sections 1-3 and OAR 410-141-3530.
