Summary
Oregon Health Plan (OHP) substance use disorder residential treatment facilities must now report all member admissions and discharges to Oregon Health Authority within three business days using secure email and the OHP 7204 form. This ensures proper coordinated care organization enrollment during treatment.
Action Required
Immediately: SUD residential treatment facilities must establish secure email reporting to ces.dmap@odhsoha.oregon.gov within three business days of all OHP member admissions and discharges. Complete and attach OHP 7204 form to each secure email report. Do not use fax for reporting due to HIPAA requirements.
