Summary
Starting March 1, 2024, Oregon Health Authority requires prior authorization using form CH-009 for all referrals to Secure Residential Treatment Facilities (SRTF), regardless of the individual's legal status. Comagine Health will review all requests for medical necessity and appropriateness before approval.
Action Required
By March 1, 2024: Providers referring patients to Secure Residential Treatment Facilities must obtain prior authorization by submitting form CH-009 with supporting documentation to Comagine Health. Download the CH-009 form from the Comagine Health website and include sufficient medical necessity documentation as outlined in the form. Failure to obtain prior authorization will result in denial of Medicaid payment for SRTF placement.
