Summary
Oregon Health Authority has extended the deadline for CCOs to submit their Community Health Improvement Plan (CHP) Progress Reports from June 30, 2024 to December 31, 2024. The reporting requirement has also been simplified - CCOs now only need to submit the completed CHP questionnaire template instead of a full progress report document.
