Update system to flag these requests and require documentation of: 1) failed less restrictive interventions, 2) FDA approval for appropriate setting, 3) caregiver training completion (patient responsibility), 4) written monitoring plan, 5) mental health management plan, and 6) transition plan away from safety bed. Submit requests to EPSDT.info@odhsoha.oregon.gov. Claims will be denied without proper prior authorization.
