Summary
Oregon Health Authority has consolidated two separate enrollment reconciliation certification forms into a single form with a checkbox to indicate discrepancies. CCOs must use the new consolidated form and updated Excel template for enrollment reconciliation reporting.
Action Required
By June 30, 2025: CCOs must transition to using the new consolidated Enrollment Reconciliation Certification form and updated Excel template available on the CCO Contract Forms webpage. Download the new form, updated guide, and template from the webpage. Previous versions will not be accepted after this date.
