Enter nursing facility's Oregon Medicaid provider number in 'Facility Number' field for all claims. Use revenue code 651 for routine home care during first 60 days of hospice election, and revenue code 650 for day 61 or later. Track prior hospice days for readmissions within 60 days to determine correct billing rate.
