Summary
Oregon Medicaid (OHA) has extended the deadline for Coordinated Care Organizations (CCOs) to submit their 2025 Patient-Centered Primary Care Home (PCPCH) annual reports from TBD to January 5, 2026. This report must include facility-level data about all members assigned to PCPCH providers in 2024 and be submitted through the CCO deliverables portal.

By January 5, 2026: If your practice is a PCPCH provider contracted with Oregon CCOs, ensure your CCO has facility-level data about assigned members from 2024 for their required report submission. Contact your CCO contract administrator if you haven't provided this data yet.
