By September 30, 2020: Billing team should complete the Provider Contact Form at carefirst.com/providercontactform to ensure CareFirst can communicate future contractual updates. Form takes approximately 2 minutes to complete. Monitor for the new Agreement documentation being mailed this month for reference.
