Starting February 1, 2021, CareFirst BlueCross BlueShield will cover remote patient monitoring for patients discharged from inpatient or emergency room stays with specific diagnoses including heart failure, chronic hypertension, COPD, chronic kidney disease, and COVID-19. The monitoring must be ordered within 60 days of discharge and requires a prescription from the discharging provider or patient's primary/specialty care provider.
