By November 16, 2021: Providers must obtain prior authorization for all ivermectin prescriptions through the CareFirst Provider Portal. Navigate to Pre-Auth/Notifications tab at carefirst.com/providerlogin to submit requests electronically. Claims for ivermectin will be denied without prior authorization approval.
