Summary
Blue Cross Blue Shield of Texas is updating prior authorization requirements for commercial members effective October 1, 2025. New codes are being added for sleep medicine, genetic testing, musculoskeletal, and medical oncology services that will require prior authorization through Carelon Medical Benefits Management, while some existing codes are being replaced or removed.

By October 1, 2025: Billing team must update prior authorization workflows to check eligibility through Availity Essentials for all services before rendering. Review the updated prior authorization lists for fully insured and ASO plans to identify which sleep medicine, genetic testing, musculoskeletal, and medical oncology codes now require prior auth through Carelon. Update billing system and encounter forms to flag these services. Failure to obtain prior authorization will result in claim denials and providers cannot bill patients.
