Summary
CalViva Health requires all providers (physicians, hospitals, PPGs, and ancillary providers) in Fresno, Kings, and Madera counties to keep demographic information current in provider directories. Providers must notify Health Net 30 days prior to demographic changes and 5 days prior to changes in new patient acceptance status, or risk being removed from provider directories.

Respond promptly to all validation requests via email, fax, or phone to avoid removal from provider directory. For PPG-contracted providers, notify PPG directly of changes.
