CBAS providers must: 1) Submit CDA forms 7012, 7013, and ADH 0006 for approval to participate in Temporary Alternative Services, 2) Maintain minimum 6-hour daily phone/email access Monday-Friday, 3) Submit weekly logs of daily participant communication to Health Net by faxing to 1-833-581-5908 every Friday with PHI cover sheet labeled 'PROTECTED HEALTH INFORMATION', 4) Document all services per existing standards and CDA guidance in ACL 20-07. Failure to obtain CDA approval will result in payment recoupment.
