Summary
CalViva Health (through Health Net) has released updated Medi-Cal prior authorization forms with auto-fill features and new address fields. The forms are now separated into inpatient and outpatient versions to simplify completion and ensure all required information is included for faster processing.
Action Required
Immediately: All staff submitting Medi-Cal prior authorizations must begin using the new updated IP and OP forms available on the Health Net provider website and Provider Library. Update workflows to use the new forms with auto-fill features and ensure provider address information is completed. Old forms will still be processed but new forms are preferred for faster processing.
