Immediately: Clinical and billing staff must verify prior authorization requirements for the following medications before administration: Blenrep, Monjuvi, Scenesse, Spravato, Tecartus, Uplizna, Viltepso, Zepzelca, Oxlumo, Breyanzi, Evkeeza, and Nulibry. Access PA request forms at Health Net provider website under 'Services Requiring Prior Authorization'. Claims for these medications will be denied without proper prior authorization.
