Summary
Health Net California changed fax numbers for medication prior authorization requests effective October 1, 2021. Pharmacy PA requests now go to 866-399-0929 and medical pharmacy PA requests go to 833-953-3436.
Action Required
Immediately: Update all prior authorization workflows to use new fax numbers - 866-399-0929 for pharmacy PA requests (medications prescribed and dispensed by network pharmacy) and 833-953-3436 for medical pharmacy PA requests (medications administered by healthcare professionals in office/hospital/infusion settings). Update fax cover sheets, billing software templates, and staff reference materials. Using old fax numbers will delay authorization approvals.
