By September 1, 2021: Home health care providers in Fresno, Kings, and Madera counties must visit Survey Monkey to complete the required DHCS Electronic Visit Verification survey. The survey takes approximately 15 minutes. Providers who render home health care services under Social Security Act section 1905(a)(7) are subject to this federal EVV mandate.
