Billing team must ensure PWK elements (PWK01, PWK02, PWK05, PWK06) are properly populated in Loop 2300 of ASC X12 837 EDI submissions. Download and use the PWK Fax Cover Sheet from CCIH provider website for all supporting documentation. Submit attachments within 10 calendar days of electronic claim submission to avoid claim rejection.
