Summary
Health Net will begin collecting medical records for HEDIS quality measures from December 1, 2021 to May 6, 2022 through their contracted vendor Datafied. Providers must respond to record requests within 7 days or complete a Certificate of No Record form.
Action Required
Starting December 1, 2021: All staff must respond to Datafied medical record requests within 7 days of contact. If records are unavailable or patient was not established during requested timeframe, complete and return Certificate of No Record (CNR) form immediately. Front desk staff should update contact information when Datafied calls to confirm before packets are sent. For EMR practices, contact Datafied at 714-666-0951 before December 1st to set up remote record retrieval to minimize in-office visits.
