Health Net/CalViva Health is conducting HEDIS medical record collection for Measurement Year 2021 from December 1, 2021 to May 6, 2022. Providers in Fresno, Kings, and Madera counties will be randomly contacted by Datafied to submit member records within 7 days of request.

Starting December 1, 2021: Medical records staff must respond to Datafied requests within 7 days by submitting requested member records or completing Certificate of No Record (CNR). Contact Datafied at 714-666-0951 if unable to meet 7-day deadline or to set up remote EMR access. Ensure contact information is current for HEDIS packet delivery.
